Abstract(in English) | We performed this study to determine whether isoflurane-induced hypotension may cause subendocardial ischemia of the heart with proximal coronary stenosis. Regional myocardial function, oxygen supply/demand, lactate production and subendocardial microvessels were evaluated under isoflurane-, adenosine- or nitroglycerin- induced hypotension (mean arterial pressure at about 55mmHg) in swine beating hearts with a fixed critical stenosis of the left anterior descending coronary artery (LD). Regional myocardial lactate production from the LAD perfused area and coronary sinus PO_2 suggested preserved myocardial perfusion with isoflurane compared to the condition with adenosine. Subendocardial microscopy revealed a phasic diameter changes of subendocardial microvessels with cardiac cycles under isoflurane or nitroglycerin, while no consistent phasic diameter changes was evident with adenosine. These findings suggest that subendocardial microcirculation is relatively preserved under isoflurane adenosine-induced hypotension, and that transmural steal does not occur with isoflurane. |
