肩甲上腕関節は複雑な運動を可能にするため,広い可動域と解剖的に不安定な構造をもつ.さらに,運動などで関節を酷使し過大な負荷を受けると関節周囲の軟部組織に障害が発生しやすい.このような障害は,肩関節の不安定性を増し,脱臼などの二次的な障害を引き起こしやすい.臨床では,肩関節障害の診断支援や治療効果を評価するため,肩甲上腕関節の接触域の定量評価が求められている.従来,肩甲上腕関節の接触域に関する報告は,主に屍体モデルを用いた手法が多い.しかし,屍体モデルでは生体内における肩甲上腕関節の運動を完全には再現できない.そこで我々は,Open MRIを用いた肩甲上腕関節の接触域の定量評価法を考案した.なお,接触域は上腕骨と肩甲骨間の距離分布とした.本報告では,考案したシステムを健常者の接触域推定に用い,その有効性を確認する.その結果,健常者の肩甲上腕関節について関節の良好な適合性を確認し,本手法の有用性を示唆した. |

The glenohumeral joint has unstable and weak structure anatomically. On the other hand, it has wide range of motion. The soft tissue surrounded on the shoulder is damaged easily by overuse during the sports activities. These injuries create more laxities of the shoulder, and cause other serious problems of the patients' shoulder. Clinically, it is very important to evaluate the contact patterns of the shoulder quantitatively to diagnose the joint disorders. There are few reports about the contact patterns of the glenohumeral joint. These reports used the cadaver models to investigate the contact patterns. However, physicians are interested in the living subject’s contact patterns. We suggested the method of estimating the articular contact patterns of the glenohumeral joint employing an open MR system. The distance distributions between the humeral head and the glenoid fossa were computed to estimate the articular contact areas. In order to validate our method, we estimated the contact areas of the normal glenohumeral joint. The results showed that the normal shoulder, the humeral head and the glenoid fossa, has reasonable congruities during the humeral elevation. |
