| 抄録 | （和） | 現在，B-mode超音波画像を用いた臨床診断は医者の読影技術に依存する部分が多く，超音波エコー信号を用いた定量診断手法の開発が強く望まれている。われわれは肝病変を中心に定量診断手法の検討を行っている。本報告では，肝炎線維化の定量評価を目的とした新しいエコー信号の解析手法について述べる。病変進行度の定量評価を行うために，病変肝からのエコー信号の振幅分布を，正常肝と線維組織からの反射信号成分に対応した二つのレイリー分布の組み合わせにより表現するモデルを提案する。また，このエコー信号の振幅分布モデルを利用して，エコー信号振幅の平均値まわりの2・3次モーメントを評価パラメータとし，病変肝中の線維組織の質と量を定量的に求める方法を提案し，臨床画像を用いた検討により，肝炎線維化の定量評価の可能性を提示する。 |
| （英） | Since the clinical diagnosis using ultrasonic B-mode image is dependent on the skill of the doctor, the realization of the quantitative diagnosis method using ultrasound echo signal is highly required. We have been investigating the quantitative diagnosis technique mainly on hepatic disease. In this paper, we present a new analysis method of the echo signal for the purpose of the quantitative estimation of the liver fibrosis. To perform quantitative judgment for the stage of liver fibrosis, we propose a model in which the probability density function of the echo amplitude is expressed as a combination of two Rayleigh distributions. These two Rayleigh distributions correspond to echo components from the normal and diseased tissues. In addition, using this amplitude distribution model of the echo amplitude, we present the estimation method to obtain the quantity and stage of fibrotic degeneration of liver from moments about mean of the echo amplitude. Basic examination using the clinical images suggested that the quantitative estimation method of liver fibrosis based on a combination of Rayleigh distributions is valid. | |
